Before your first visit, we ask that you take time to fill out the following new patient form and past & current condition form. Also, take a few minutes to watch the video below, where Dr. Luke will take you through what to expect over your first several visits.

Appointments

The time of your appointment has been reserved exclusively for you.  Please notify us at least 48 hours in advance by phone if you are unable to keep appointment.

Payments

Charges are payable at the time your treatment or service is rendered. Regardless of your insurance coverage, our office relies on you to settle your account. For your convenience we offer the following payment options:

Insurance

Pantalone Chiropractic does not participate with any insurance provider, but we will submit to insurance on your behalf.
